Our Katy dental office is all about comfort and convenience. That’s why we offer our patient forms to fill out online, or download and fill out at home. Be sure to either submit online by clicking the “Online Forms” link below, or bring your filled out New Patient forms to your visit and your child will  be in and out in a flash!

If you are unable to print or fill out this paperwork at home, please arrive 20 minutes early to your first appointment. This information lets us know a little more about your child, their dental needs and insurance information. Plus, it helps us give you the best care possible!

To Parks Youth Ranch

Pediatric dentist in Katy wholeheartedly believe in giving back to the community that serves us so well. That’s why we’re proud to offer our “Pay It Forward” program! It’s designed to give a portion of profits from all new patient exams to Parks Youth Ranch, a nonprofit focused on providing a stable temporary home for displaced children ages 7 to 17.

As a new patient, you can take part in helping kids who have been abused, abandoned, or neglected. Your donated dollars help provide a safe and supportive environment while a child waits for their new home. Give us a call and make an appointment today!
